In order to make an appointment, email us using the hyperlinks below or you may call the individual locations at the bottom of the page. If booking by email please provide the following:

• Desired appointment date and time, with approximate number of participants.

• Company name, complete company address, phone number, fax number, email address and contact person's name.

We will confirm your appointment via email or phone.  You will receive an announcement poster in the mail, prior to your appointment, to post for the employees' review.  Please encourage your friends and family join in your appointment or to contact us for the nearest location so they, too, may avoid the headache of getting a last minute flu shot this fall.
